    Abstract:

    OBJECTIVE: To determine the efficacy of combined l-carnitine and l-acetyl-carnitine therapy in infertile males with oligo-astheno-teratozoospermia. DESIGN: Placebo-controlled double-blind randomized trial. SETTING: University tertiary referral center. PATIENT(S): Sixty infertile patients (aged 20-40 years) with the following baseline sperm selection criteria: concentration, 10 to 40 x 10(6)/mL; forward motility,<15%; total motility, 10% to 40%; and atypical forms,<80%. Fifty-six patients completed the study. INTERVENTION(S): Patients were submitted to a combined treatment of l-carnitine (2 g/d) and l-acetyl-carnitine (1 g/d) or of placebo; the study design was 2 months' wash-out, 6 months of therapy or of placebo, and 2 months' follow-up. MAIN OUTCOME MEASURE(S): Variation in the semen parameters that were used for patient selection. RESULT(S): Even though increases were seen in all sperm parameters after combined carnitine treatment, the most significant improvement in sperm motility (both forward and total) was present in patients who had lower initial absolute values of motile sperm (<4 x 10(6) forward or<5 x 10(6) total motile spermatozoa per ejaculate). CONCLUSION(S): Combined treatment with l-carnitine and l-acetyl-carnitine in a controlled study of efficacy was effective in increasing sperm motility, especially in groups with lower baseline levels.

    Abstract:

    Aluminium (Al) has been proposed as an environmental factor that may contribute to some diseases, affect several enzymes and other biomolecules and induced free radical-mediated cytotoxicity. Also, Al induced reproductive toxicity and exerted a significant adverse effect on the steroidogenesis. The antioxidant ascorbic acid (AA) plays an important role in various physiological processes in the body including detoxification of different toxic materials. Therefore, the present investigation aimed to elucidate possible protective effects of AA in alleviating the toxicity of aluminium chloride (AlCl3) on reproductive performance, lipid peroxidation and enzyme activities in seminal plasma of male New Zealand white rabbits. Six rabbits per group were assigned to one of four treatment groups: 0 mg AA and 0 mg AlCl3 /kg body weight (BW) (control); 40 mg AA/kg BW; 34 mg AlCl3 /kg BW; 34 mg AlCl3 plus 40 mg AA/kg BW. Rabbits were orally administered their respective doses every other day for 16 weeks. Results obtained showed that AlCl3 significantly (P<0.05) decreased libido (by increasing the reaction time), ejaculate volume, sperm concentration, total sperm output, sperm motility (%), total motile sperm per ejaculate (TMS), packed sperm volume (PSV), total functional sperm fraction (TFSF), normal and live sperm and semen initial fructose. While initial hydrogen ion concentration (pH) and dead and abnormal sperm were increased (P<0.05). Live body weight (LBW), feed intake (FI) and relative weights of testes (RTW) and epididymis (REW) were significantly (P<0.05) decreased. Concentrations of thiobarbituric acid-reactive substances (TBARS) were significantly (P<0.05) increased in seminal plasma of rabbits treated with AlCl3 compared with control. While, activities of glutathione S-transferase (GST), aspartate aminotransferase (AST), alanine aminotransferase (ALT) and acid phosphatase (AcP) were significantly (P<0.05) decreased. Ascorbic acid alone significantly increased LBW, FI, RTW, REW, semen characteristics and seminal plasma enzymes, and decreased the levels of free radicals. Also, the present study showed that ascorbic acid might be effective in the protection of aluminium-induced reproductive toxicity. It was suggested that AlCl3 exerted a significant adverse effect on reproductive performance of male rabbits. Furthermore, AA could be able to antagonize the toxic effects of AlCl3 and improved semen quality of male rabbit.

    Abstract:

    Aflatoxins are toxic to a wide variety of animals, including man. The antioxidant ascorbic acid (AA) plays an important role in various physiological processes in the body including detoxification of different toxic compounds. The objective of this study was to evaluate the effects of AA on productive and reproductive characteristics of mature male rabbits given two sublethal doses (15 or 30 microg/kg of body weight; every other day) of aflatoxin B(1) (AFB(1)). The experiment lasted 18 weeks and included two periods: a treatment period (first 9 weeks) where the animals were given the tested materials, and a recovery period (second 9 weeks) where all the drugs were withdrawn. Results showed that live body weight (LBW), dry matter intake (DMI), relative testes weight (RTW), and serum testosterone were significantly reduced (P<0.05) by treatment with AFB(1) in a dose-dependent manner, and these effects continued during the recovery period. Aflatoxin treatment also decreased (P<0.05) ejaculate volume, sperm concentration, total sperm output, sperm motility index, and semen initial fructose concentration. The negative effects of aflatoxin on semen characteristics were dose-dependent and continued during the recovery period. Treatment with AA increased (P<0.05) LBW, DMI, RTW, serum testosterone concentration, improved semen characteristics, and alleviated the negative effects of AFB(1). Aflatoxin treatment increased (P<0.05) the numbers of abnormal and dead sperms in a dose-dependent manner, and this effect continued during the recovery period. Treatment with AA alleviated the negative effects of AFB1 during treatment and recovery periods. Results demonstrated the beneficial influences of AA in reducing the negative effects of AFB(1) on production and reproduction of male rabbits.

    Abstract:

    AIM:Among the factors contributing to male infertility, asthenospermia constitutes both a health and a social problem frequently associated with alterations in sexual function. Studies have shown that acetyl carnitine and L-arginine improve sperm motility and that ginseng enhances libido and sexual performance. This study examined the effect of treatment with carnitine, acetyl carnitine, L-arginine and ginseng in men with idiopathic asthenospermia and altered sexual function.

    METHODS:The study population was 180 patients with asthenospermia randomly assigned to two groups: group A (90 men) received treatment and group B (90 men) did not. The sperm count was 16.6± 3.2 x 10⁶/mL and the total sperm motility was 26.5 ± 3.4%.

    RESULTS AND CONCLUSION:Sexual satisfaction was measured using the sexual satisfaction index (SSI). At the end of therapy, a significant improvement was observed in progressive sperm motility on spermiogram evaluation and in SSI scores in the treatment group.
